Unani medicine in India was introduced in 1351 AD by Arabs. The first known Hakim was Zia Mohd Masood Rasheed zangi. Unani Medicine soon got acceptance by the masses due to its efficacy and nontoxicity of the drugs. I would like to know the present status of Unani medicine in India and also the details about its diagnosis process and treatment....

Unani medicine is ancient Greek medicine that has evolved within the Muslim world for the past 13 centuries Unani is an Arabic spelling of Ionian, meaning Greek . Greek medicine, greatly simplified for presentation here, was based on the concept of balancing body humors. They either fell out of balance, which might yield diseases depending on circumstances , or were restored to balance to heal diseases. The system involved four elements, thus differing from the Ayurvedic system of three doshas and the Chinese system of five elements. The original Greek and the resulting Unani systems involve these four elements: earth, air, water, and fire along with four natures: cold, hot, wet, and dry and four humors: blood which is hot/wet , phlegm cold/wet , yellow bile hot/dry , and black bile cold/dry .

Hakim Abdul Majid 1883-1922 founded an organization called Hamdard in Delhi in 1906 at the time, it was a small clinic and herbal medicine shop. Abdul had come from a family that included many herbal doctors, and he joined the herbal pharmacy of the renowned Unani physician Ajmal Khan. As he developed his knowledge of medicine, he too became a doctor Hakim and decided to establish his own pharmacy and clinic, which he called Hamdard Dawakhana. Hamdard, meaning sympathy, refers here to one who shares the pain of others and is willing to mitigate it Dawakhana means clinic . As a result of Hamdard s success, it was moved to a new larger building in 1922 unfortunately, Abdul suffered a serious illness and died that year. The responsibility for Hamdard fell to his wife and his eldest son in a family of five children , Abdul Hamid, who was only 13 at the time and already studying medicine. Hamdard would undertake its biggest growth and development once Majid s youngest son, Mohammad Said, agreed to study medicine he had originally wanted to be a journalist . In 1940 he undertook studies of both Ayurvedic medicine and Unani medicine. After graduation, he participated in the Hamdard pharmacy and clinic with his older brother. Soon, however, the state of Pakistan was formed and many Muslims from India migrated there, including Mohammad Said, who was a devout Muslim. In 1948, he moved to Karachi, and set up a branch of Hamdard there, with both clinical facilities and herb manufacturing. In keeping with its aim to serve the people, it was turned into a charitable trust in 1953. Meanwhile, the original facilities in India, devoted exclusively to Unani medicine, continued to expand under its director, Hakim Abdul Hamid, it became world famous and is likely the largest facility on earth for research and training in herbal medicine.

India has 18 colleges of Unani medicine, two of them with postgraduate departments one being Hamdard Tibbi College in Delhi see Appendix 3 about another major contributor to India s Unani system . The annual new admissions in the undergraduate courses is about 700 students, and 30 students in the postgraduate courses. A National Institute of Unani Medicine is in Bangalore. There are about 100 Unani hospitals in India, with nearly 1,400 beds for inpatients. In addition, there were nearly 900 dispensaries of Unani medicines and about 30,000 registered practitioners.

Unani medicine was originated in Greece and its founder was great philospher & Physician ,Hippocrates Buqrat 460-377 BC .He was the first Unani Physician who opened the education of Medicine to all communities, so he is known as the father of medicine in Allopathic also because modern medical science was developed on the foundation of Hippocratic philosophy of health and disease. Before Hippocrates it was restricted to the Aesclabius family only. After Buqrat, Galen Jalinoos130-201 BC contributed a lot to the Unani Medicine. Aristotle Arastoo384-322 BC laid down foundation of Anatomy & physiology. Dioscorides was the famous physician in the Ist Century AD. He was famous for his book on Ilmuladviya Pharmacology named as Kitabulhashayash. In Egyptian period the pharmacy was very much developed and they prepared the medicine in different dosage forms like alcohol, oils, powder and ointment etc. Persia encouraged and developed physicians and philosophers by the Iranian .They further developed this medicine into a complete science and it got deep roots in the masses. Eminent Physician of this period are:- Ibne Raban Tabari 810-895AD was famous physician and wrote a book Firdous ul Hikmat and introduced concept of official formulary. Abu Bakar Zarakariya Razi or Rhazes 865-925 AD etc. He wrote the book Alhawi fit tibb . He was first scholar who described the concept of Acquired Immunity by his experiments in the article Maqala fi Judri wa Hasba .The Islamic rulers of the Arabian countries further developed and adopted the Unani Medicine at government level and here at first they recognised the importance of the official formulary Qarabadeen . They made many additions /deletions on the basis of their experiments, advance knowledge of Medicine in Greek and Persian literature. They developed the chemistry, pharmacy and pharmaceutical procedures. In this period they invented the process of distillation, sublimation, calcination and fermentation to promote the efficacy of the medicine and to remove the impurities and toxic effects of the drugs. Jabir bin Hayyan 717-813AD who was settled in the Arab as Royal physician was a famous physician and Chemist. He was the first who had described Chemistry. The very eminent scholar of this period was Bu Ali Sina Avicenna 980-1037AD . He was the great philosopher, physician of the Arab. He gave a final shape to the Unani medicine & redefined many concepts based on his clinical experiences. His book Alqanoon or The canon of medicine was an internationally accepted book on medicine, which was taught in European countries till the 17th century.

The Arabs have introduced Unani medicine in the Spain in the patronage of Spanish Ruler, many scholars contributed a lot to Unani Medicine for instance 1. Abul Qasim Zohravi Abulcasus 946 -1036AD was the famous surgeon was attached to Royal Hospital of Cordoba. He wrote a famous book on surgery Al Tasreef . Ibn-e-Haisham 965-1039AD was a renowned scientist and Unani physician from Syria laid down the foundation of optics in his famous book Kitabul Manazir . Ibn-e-Zohar Avenzor1091-1162AD was a renowned physician and Scholar. He was known for his famous book Kitabul Tasreef . Ibn Rushd 1126-1198AD was famous for his book on Basic Principals of Unani Medicine known as Kitabul Kulliyat. Ibne Baitar 1197-1248AD was Pharmacognosist and Botanist. His famous book on extensive survey and experiments on Single Drugs of Plant origin Kitabul jameul Mufradat. Was widely acclaimed. Ibn-e-Nafees 1210-1288 AD was the first scientist who described the blood pulmonary circulation and prescribed anatomy as a separate chapters. He was known for his book Qarabadin Qadri and Tibbe Akbar,based on his own lifelong clinical experience. Hakim M. Shareef Khan 1725-1807 A famous Hakim of Delhi in Mughal Period. He was famous for his book Ilaj ul Amraz. Hakim Ajmal Khan 1864-1927 Hkm Ajmal Khan. was renowned Hakim,scientist,politician and freedom fighter and a great patriot. He has established Unani and Ayurvedic College at Karol Bagh Delhi. He is remembered as Masihul Mulk Hkm Ajmal Khan.He was the first person who opened the door of research in Indian Systems of medicine and under his supervision Asarol Rauwolfia serpentina was analysed and isolated various alkaloids i.e. Ajmaloon, Reserpine etc and most effective drug for Hypertension was introduced in world. Hakim kabeeruddin April 1894-9th January 1976 was very dedicated scholar of Unani Medicine . He was translated 88 Unani books of Arabic and Persian languages into Urdu. Which was the greatest achievement. Due to this achievements today, the Unani colleges in India are in existence. The first institution of Unani medicine was established in 1872 as Oriental College at Lahore. Thereafter many institution came into existence. Post independence India period After independence Govt. of India at first constituted Unani pharmacopoeia Committee in 1964.,consisting of Unani experts and scientist with a view to maintain uniformity in the standards of drugs and to prescribe standards for compound formulations. And also to prescribe tests for identity, purity, efficacy and quality of the drugs. Pharmacopoeia Laboratory of Indian Medicine at Ghaziabad was established under Govt. of India to workout standards and drug testing for Indian System of Medicine at national level. Govt. of India has also established Central Council of Indian Medicine ,an statutory body,for laying down standards and maintaining uniform standard of education in the Indian System of Medicine including Unani and to regulate the practices in Indian System of Medicine. In order to develop high standards of education and research in Unani Medicine at national level a National Institute of Unani Medicine has been established at Bangalore in 1983 in collaboration with the Govt. of Karnataka which provided 55 acres of land for the Institute. In addition to these developments Central Council for Research in Indian Medicine was established in 1969.During 1978 these these Council were bifurcated into separate Central Councils as such Central Council for Research in Unani Medicine was established in 1979, with a view to initiate ,guide, aid, develop, coordinate scientific research in different aspects of fundamental and applied Unani System of Medicine. In the combined Council Hk. A. Razzaque was the Head of the Unani Council. He was the expert and educationist who has established different wings of the present council all over India. As such there are 11 Regional Research Institutes, 7 Clinical Research Units, one Literary Research Units, 2 Survey and Cultivation of Medicinal Plants Unit, 7 Drug Standardisation Research Unit.

Unani medicine in India originated with the coming of Arabs. Historical evidences speak that this medicine was used by the Delhi Sultan, the Khiljis, the Tughlaqs and the Mughal Emperors. During the 13th and 17th Century Unani Medicine was practised profusely in India. Unani medicine in India was used to treat smallpox and measles. Unani medicine mainly diagnoses the pulse nabz besides an examination of the urine baul and stool baraz . While examining the pulse, numerous aspects are looked for, like volume, tension, blood volume, pattern of movement, rest, rhythm, irregularity and so on. Treatment is done for simple diseases and it is mainly through diet in the initial stages, followed by the administration of a single drug, failing which compound preparations may be administered. Each drug is placed under four categories, based on potency and efficacy. There are four treatment therapies employed in Unani medicine in India. These includes regimental therapy venasection, diuresis, Turkish bath, massage, purging, exercise and so on pharmaco-therapy, use of herbal, animal and mineral drugs surgery and dietotherapy which quantity and quality of food. Whereas for diabetes, several bitter and astringent items such as stones of the jamoon fruit, phalsa fruit, bittergourd juice, tender neem shoots, bilva leaves and cottonseed kernels are prescribed by the Unani medicine in India

About Unani Unani means medicines which are a symbol of life. The name is derived from the word Ionian which originated in Greece. Tibb means the knowledge of the states of the human body in health and decline of health, or in other words, medicine. Tibb-E-Unani , is hence an age old, time tested system of medicine dating back 5000 years to Greece. Unani medicine, like any other form of medical science strives to find the best possible ways by which a person can lead a healthy life with the least or zero sickness . It prescribes drugs, diet, drinks and other regiments including codes of conduct which are conducive to the maintenance and promotion of positive health, as well as the prevention and cure of disease. The ultimate aim of these scientific prescriptions and prohibitions is the creation of a healthy society. Traditional systems of medicine are no doubt prevalent in various countries of the world. But most of these are empirical. On the other hand, Unani and its allied branches have rational and scientific basic principles. It is distinct from other branches of medicine, as the drugs it uses are natural in their sources and forms. It emphasizes on retaining natural compounds which belong to the human body, and hence prescribes only natural remedies. Unani medicine believes that diseases can be kept at bay by the use of clean and fresh water, breathing clean air and consuming fresh food. Likewise, a balance should be maintained between the mind and the body so that the metabolic process can take place easily and the body waste evacuated. Unani medicine also believes that all life forms have originated from the sea. There are eight specialized branches of Unani medicine - Internal medicine Moalijat Gynecology including Obstetrics and Pediatrics Diseases of the head and neck Toxicology Psychiatry Rejuvenation Therapy including Geriatrics Sexology Regimental Therapy Dietotherapy Hydrotherapy Historical background of Surgery in Unani Medicine : - For historical reasons, the practice of surgery was strongly discouraged in the medieval period. But in ancient times Unani surgeons did perform intricate brain surgery, laparotomy and plastic surgery. Inspite of the apathy generated by colonial rule, the faith of the people in their culture and traditional medicine kept the remaining branches of Unani alive. It is because of this mass base and utility that after Independence, the Government of India took several steps to further this health science. Hippocratic Theory of Medicine : - The first fundamental theory of the Unani system established by Greek philosopher Hippocrates is founded in the belief that the body of the individual is composed of four basic elements, which together, are called Anasir-e-Arba Hava, Pani, Mitti, Dhup . In different permutations and combinations, these four elements or biological fluids constitute Humours Akhlat i.e. blood or Dam , phlegm Kafa , bile Safra and black bile Souda . Thus, the basic principles of Unani is that of the four humours represented in the human blood. As long as these humours exist in normal balance, normal quantities and in the normal region of the body, the humour system will work in the normal way. Any imbalances to the humour constitutions or changes in their quantity and quality result in diseases. Later, several other scholars improved and expanded the scope of the Unani system. According to the Unani discipline as it stands today, the human body is composed of 7 natural and basic components called Umoor e Tabaiyah which are responsible for maintenance of health. These are : - Arkan Elements Mizaj Temperament Akhlaat Humours Aaza Organs Arwah Vital forces or Neuro Quwa Faculties Afaal Functions The loss of any one of these basic components or alteration in their physical state could lead to disease, or even death. It is highly essential to consider all these factors so as to reach the correct diagnosis and consequently the correct line of treatment. The Unani physicians describe that there is a special ability hidden in every individual called the defensive mechanism of the body or in the language of Unani - Tabiyat-e-Muddabare Badan . The Tabiyat may be defined as the sum total structural function and psychological character of the human being. The Tabiyat or nature is the best physician, and maintains the equilibrium of the four humours or Akhlat . Their quantity and quality should be as per the natural chemical composition of the body. The UNANI System of addressing whole health considers illness as an event serving to cleans, purify, and balance us on the physical, emotional, mental, and spiritual planes. Allah The one God of All says in the Quran There may be a thing decreed for you that you do not like that is good for you and things that you like that are not good for you. Imam al-Ghazzali r.a. expressed this idea as follows: Illness is one of the forms of experience by which humans arrive at a knowledge of God as He says, Illnesses are my servants which I attached to My chosen friends. Just because something feels good does not necessarily mean it is of ultimate benefit to us. And conversely, if we have a moment of pain, we cannot dismiss this experience as bad without understanding how and what the result of these sensations will be. A Healing Crisis is the natural function of the body eliminating toxins: Vomiting, Nosebleeds, Diarrhea, Fevers, Sweating, and Frequent Urination these are avenues to full health. While admitting the existence of microbes, the UNANI Tibb system maintains that it is the original imbalance of temperament that provides an altered biotic environment in which the viruses and bacteria can thrive. Treatments of killing off microbes can provide a temporary cure , but without restoring the humor to its proper balance, the disease will recur. So, the UNANI Tibb System is one of seeking balance between the Humors of the body and the temperament, mainly through proper metabolism of the bodies nourishment.
